Forms/Permission Slips
There are a number of forms (medical release, etc) that are required by the diocese. Often this means filling out numerous forms (at times repetitive) for each event. This can be time consuming but is being done for the safety of your child. Click on the forms icon below and you will be able to download the forms. Extra copies can also be obtained on the door to the Youth Ministry Office
